Items lost on death

Suggestion: Outside the wilderness we keep pkh way, in the wilderness it'll be the osrs way?

Is there a way to structure it such that if your death is to NPC's it'd be the second option, but the first if it's to a player?

Technically, yes, but it would be preferable to have the same mechanic no matter how you die. If the vote ends in a tie, I might consider it, but I'm hesitant to make it that way.
Not to mention that the "items lost on death" screen would be incorrect, leading to added confusion. Or we'd have to add "Show items lost on death in PvM" and "Show items lost on death in PvP" options when you click the items lost button in the character overview tab.

The armor. Have you never played RS before? It keeps the top 3 items with the most value, or 4 with protect item. Idk why this concept is so difficult for you to grasp, it's really simple actually.

Ergo, you will not keep it. You will only keep up to 3 (or 4) of that item if it is within your protected items.

Say you have 2000 100m tickets with you while wearing bandos chestplate, bandos tasseys, perfect ring, and chaos tent. Then, oh no, you die. With item protect on, you will keep bandos chestplate, bandos tasseys, perfect ring, and chaos tent because all 4 of those items are worrh more than a single 100m ticket. If you had 2000 100m tickets while wearing full rune and a rune scimitar, you will keep 4 100m tickets on death (with item protect).

That's the way it should be, you can easily get all your items back at any npc (gwd with Ec key).
